Most people get tattoos or piercings, experiment with hair and nails, or even change their sex and gender.

Imagine being so empty and vapid that the form your body modification takes is this garish, unnatural, conspicuous plastic surgery that doesn’t even achieve the desired effect of youthful appearance, but instead signals to other rich people that you have disposable income like an expensive watch or a fast car would, except with none of the aesthetics and all the fun of constant surgery.

Terry Gilliam misses a lot, but he hit the nail on the head with that face surgery stuff in the movie Brazil.

Also I've always found this look repulsive not because of anything to do with cultural associations but because it triggers my disease avoidance instincts. The same way you don't go sticking your face in roadkill, you don't go kissing someone that looks like they're suffering from a severe facial or mouth infection.
