I did a bit of research regarding the CIA drone-strike program in Afghanistan. It ran about five-hundred sorties a year. The approximate rate was fifty civilians killed for every person-of-interest. Drone operators called casualties bugsplats. They called children fun sized terrorists. This was the lingo in used in the operation. When reported to the press, dead people, men, women, children, all were reported as militants.
Drone-pilot burnout was epidemic in the program. It turns out soldiers get PTSD from killing people whether it's up close or thousands of miles away. And often the drone would lurk for a while, watching all these people eat, work, play, sleep, fuck (they had IR and could see into buildings), and would clearly understand these were just people living their lives when the order would come down to incinerate them.
And this wasn't under George W. Bush (who still wiped out villages, but used PMC raiders doing black ops) this was under Obama. The disposition matrix and the FISC decided whether a POI needed to be executed. They didn't consider the people that lived at the target site.
And that's just the Afghanistan program in the International War on Terror. There was also a Pakistan program.
The United States in its military operations has been really merciless, and at some point we need to collectively decide if we're okay with that, or are driven to change.

The article states that Palantir's Maven system was over-relied on, and compressed a process that would have taken months into one that took days, but it also points out that that Maven didn't actually make a mistake based on the way it was supposed to operate.
It sounds like some military personnel were mistaken in how much vetting and revetting maven itself would do, but the article also lays much of the blame at Hegseth's gutting of the civilian harm roles, and failures in intelligence vetting and gathering that fed into maven in the first place. Years ago US intelligence personnel had flagged that Minab had turned into a school, but that information was not in the database used by maven.
Like I have no doubt that Maven is part of the problem, but it sounds like it was far from the only part.

So who is ultimately responsible? AI isn’t something like an unfortunate mechanical failure (though even than someone bears responsibility). If the government is using AI for targeting military attacks, it sure better understand every single risk associated and be ready to swallow every ounce of responsibility. Meh, I was having an argument with a coworker recently about AI's evils, and all the doomsayers and apoca-hype profiteering that the big AI companies do. I think this article lays out more plainly what the risks of AI are in terms of ending the human race, than any of the claude-cult stock pumpers that come forward.
It won't be some AGI super intelligence that ascends and then does away with humanity. It'll be some random military sorts who get a prompt, and trigger a huge conflict / disaster based on bullshit fed to them from an AI. The AI won't even do it 'intentionally', it'll just be a convincing enough marionette's lie, fed to a low enough IQ, to open fire.
