In larger US states, yes. Typically north/northwest, like in Upstate New York.

Depends on where you are when you say it. To anyone in NYC, that's probably true. But don't say you're in upstate NY if you're in western NY.

There is only really "Upstate", New York, and "Long guy land".

In this context it probably just means from the lowland of the state to the mountains.

Based on what? That's not how "upstate" is used... like ever. It's not synonymous with "upstairs."
