I think it's a flaw in the nature of Lemmy, and there might not be anything that can be done about it.
If there's a photo of a naked blonde twisted with her ass and chest in view taken in public, it'll get submitted to:

The solution I was presented with last time I mentioned it on Lemmy was to block the communities or posters in question, which is a ridiculous idea.
