Unfortunately, the hospital bed photo looks real and the vacation photo is fake. Note the 2 light sources reflecting off the turtle's shell. They make no sense outside, but they match the light sources in the hospital photo.
Honestly, I think both are fake, but the vacation one looks more natural for their bodies. Her arm in the hospital photo is funky and so is his. Also, he's way too young looking.

I think her arm is like that because she's trying to make sure he doesn't fall over.
Despite the poor resolution, you can still kind of see his hospital bracelet in the vacation photo.
Some phones do that, every photo my sister takes if the text isn't the focus it looks like ai crap.
to me what looks weird in the vacation pic is the alignment of torso and legs. It looks really off. In the hospital pic is more natural. So I'm inclined to agree with you that hospital is real and holiday isn't.
Also, I was looking for an actual turtle in the holiday pic to check reflections, it took me a while to understand 😅
I saw the vacation pic posted earlier in a now-deleted post that tried to pass it off as real. When asked for a source, OP admitted that they didn't care if it was fake or not, and not long after it was taken down. As funny and damning it would be if they just reused an old photo as proof of life, I highly doubt the vacation pic is legit.
That said, the hospital one is fishy as fuck imo
That's what I think too, that they're both fake. Both are freaky bad.
