They survive by firmly staying in denial about their own pain, shame and insecurities. And they do that by offloading and projecting those things to the people around them, with zero regard for how that might affect them.

I'm going to reserve my empathy for the people who have to deal with narcissists rather than the narcissists themselves.

I agree, and especially when we're talking about adults -- we are always responsible for our own actions. Those around us are not responsible for us or for managing our emotions or tip toeing around our shame. Projection is one of the really toxic parts of dealing with a narcissist. They attempt to make others responsible for what belongs to them.
