That is actually surprising to me. I guess I'm still too hopeful that behavior matters to voters, and that negative actions have negative consequences.

Not sure if you're just referencing whatever this image is from but if you're serious, the answer is alcohol and a sedentary lifestyle
I meant the question literally. I know a lot of alcoholics, and none of them look like that. Is it from swollen organs?
I realized I didn't actually know so I just looked into it and apparently it's a lot of things. But the gist of it is that alcohol abuse fucks up your intestines which leads to bloating. Also alcohol has a lot of calories
