Where does this "and all I got is this lousy x" phrase and t-shirt format come from? I know Vice City gave you one for completing it, but was it the first?

I think it predates the Internet. Definitely predates Vice City. Tourist shop stuff.

It was a souvenir t-shirt trend in the 70's. Some sites claim it is from [a line of dialogue in] the movie M*A*S*H but that's not true, which is easily provable by searching through the subtitles for the movie.
