Can a radiologist help me understand some of the more interesting differences between the two?

Not a radiologist but I wanted to make clear that these are two different women. You can see there is a height difference by looking at the ankle bones. The woman on the right is several inches taller.

She also appears to be standing on her tiptoes.

I am a very obese man. My current doctor hasn't stated this outright but my previous doctor did (politely) and observed that, as people gain weight, they tend to angle their feet outwards for balance. I wonder if that's related to what's happening here.

Probably laying down, not standing on their tiptoes

That is exactly what is happening here.

The hip bones also appear to be angled differently too, as if her legs are rotated, which could also explain the ankle?

Though not the knees apparently.
