Younger Shaw was going through a lot while in Axis Mundi, so he may have convinced himself that he was hallucinating or something like that.
As far as Isabel, Apex was probably keeping an eye on Hiroshi, especially after he left Monarch. They may have known he disappeared and they had no reason to tell anyone.
She also did admit to doing some stalking of Kentaro, so she probably overheard it, or Hiroshi may have actually filed a report or such during the 2 years Cate and Company were gone.
