Maybe you had 10 luck + a luck bobblehead? They're easier when you have better luck stats
I always run a high luck build, and I am persistent. My last playthough I got the pie from every vending machine I found, and kept track of how many tries it took. Best was 4, worst was 88 or 89 (I may have missed one, and put an asterisk in my notes to that effect)
You probably missed the one in the junkyard since that one always, no matter the luck you have, gives you a perfectly preserved pie, that's probably why it's in the junkyard lol, that's my favorite piece of world building in the game
I always try the one at the Dugout Inn and the one on the way to robbing Hancock with Bobby No-Nose. Only once have I gotten it. I've only got about 1000 hours in FO4 though.
Is this actually just a low-incidence thing or is it a mod?

The % chance of success is (luck x 4) + # of attempts / 20. You’ve got about a 50% chance of getting 1 in 50 attempts with a Luck of 1.
You can sit there and just keep trying but that’s no fun. I only try once at each Port-a-diner. As a good luck charm.
If you have 10 luck you have a 100% chance of getting it within 1960 attempts.
No mods. I mean, yes there are mods installed, of course, but none that affect the chances of this happening.
I got one once too! The actual item was really underwhelming though which was sad.
I have never interacted with those things.
Gimme That Pie! is always installed because I must have all the pies.
